
New program introduces physician-led, data-driven model to improve chronic disease management, reduce medication burden, and enhance quality of life in assisted living residents
TOWARD Health is excited to announce the launch of a novel assisted living initiative aimed at transforming chronic disease management through structured, evidence-based metabolic care. The program is supported by a philanthropic grant from the Baszucki Group and will be implemented in collaboration with A Paradise for Parents, an assisted living network based in Arizona and led by Hal Cranmer.

The initiative introduces a coordinated care model designed to address the complex medical needs of assisted living residents, many of whom live with chronic conditions such as type 2 diabetes, cardiovascular disease, obesity, and neurodegenerative disorders. By integrating metabolic medicine, geriatric care, and lifestyle-based interventions, the program seeks to improve clinical outcomes while reducing reliance on medications and fragmented care delivery.

Through its partnership with A Paradise for Parents, TOWARD Health will implement this model in real-world assisted living settings, working closely with on-site caregivers, staff, and leadership to ensure seamless integration of clinical protocols and care coordination. The initiative is structured as a clinical quality improvement program, implementing established standards of care with an emphasis on metabolic health.
Program Overview
The TOWARD Health assisted living model combines telehealth-enabled physician management with on-site interdisciplinary care teams. Core components include:
- Personalized therapeutic carbohydrate reduction tailored to individual clinical needs
- Continuous metabolic monitoring, including glucose tracking when appropriate
- Structured exercise programming to support mobility and functional independence
- Medication reconciliation and targeted deprescribing strategies
- Integrated coordination across nursing, dietary, and rehabilitation teams
This approach is designed to bring consistency, precision, and accountability to chronic disease management within assisted living settings.
Data-Driven Evaluation and Scalability
The program will systematically evaluate clinical and operational outcomes using data derived from routine care, including metabolic markers, functional assessments, cognitive screening, medication utilization, and resource allocation.
Initial implementation will focus on a diverse group of residents with varying levels of independence and medical complexity, including those living with metabolic conditions, cardiovascular disease, and cognitive or neurologic challenges. Insights from this program will help develop a scalable model that can be expanded across additional assisted living communities.
